Physiotherapy Rehabilitation Case Management Services
At Neurological Physiotherapy we recognise that clients with an assigned case manager typically have complex and multifaceted needs. We provide expert physiotherapy treatment for both neurological and musculoskeletal presentations, and collaborative support within multidisciplinary teams across legal, health, education, independent, and statutory services.
Our practice manager will liaise with the case manager as a first point of contact, providing terms and conditions and costings to secure initial funding.
Case managers can then expect their client to receive a holistic assessment with one of our physiotherapist best able to treat their client’s specific presentation. This assessment can take place at our specialist clinic or at a community location. A full assessment report and costings for the recommended treatment plan will be provided.
Following agreement of the treatment plan and funding, the case manager can expect regular updates including quarterly or bi yearly review reports depending on the client’s rehabilitation needs. In instances of treatment ending, a full discharge report will be provided.
